Answering this and other seemingly straight-forward related questions led to complexities that no one was expecting. This exercise revealed to senior clinical and technology management that there was no single record of each patient’s complete care experience, and their varied clinical encounters with individual healthcare providers were being recorded in assorted and dissimilar ways, leaving inconsistencies in the data and obscuring insights into the nature and value of important patient-provider relationships.
